Employee Start Pack
The employee start pack contains information to read and all of the forms that must be completed and returned in order for new employees to begin receiving a paycheck.
The start pack includes W4, Direct Deposit and weekly time sheets forms.
Once the forms are completly filled out, all must be returned either by fax at (630) 468-1997 or by mail.
